At the time of testing, it is recommended that physicians/practices complete this CDC form, which includes basic information about the individual. There is not clear direction if the form needs to be faxed to your local health department for every test, but it MUST be sent if a test is positive. Use the secured fax or phone line you would normally use to report communicable diseases. Contact your local health department for guidance regarding form submission for all COVID-19 testing.
